Hazel Mae Forman Pleads Guilty to Operating Unlicensed Care Home
Nearly a year after 74-year-old Larry Darnell Burton mysteriously vanished from a Greensboro adult care home that state officials later determined was operating without a license, the woman accused of running the facility has pleaded guilty in Guilford County District Court.
Hazel Mae Forman, 50, entered a guilty plea Monday to a misdemeanor charge of operating an adult care home without a license. But while Forman’s criminal case may be over, the mystery surrounding Burton’s disappearance remains unsolved.

Court records show Forman pleaded guilty on June 15, 2026, to a single count of operating an adult care facility without the required state license.
The charge is classified as a Class 3 misdemeanor under North Carolina law.
Following her plea, a judge ordered Forman to pay a $50 fine along with $183 in court costs. She was not sentenced to jail time and was not placed on probation.
The Disappearance of Larry Darnell Burton
Forman operated what was known as L&L Family Care Home (also known as HMF Family Care Home) in the 1200 block of Moody Street in Greensboro, near East Gateway Boulevard and areas like Barber Park. State investigators from the North Carolina Department of Health and Human Services (NCDHHS) confirmed the facility was running without the required license to provide adult care services.
Larry Burton was transferred to the facility from Moses Cone Hospital around June 24, 2025, for post-hospital care. His family, including daughter Latonya Swift, publicly shared concerns about the circumstances of his placement. According to our previous reporting, hospital staff reportedly would not discharge him home due to his medical needs and “combative” label, despite family caregiving history.
Insurance issues reportedly played a role in directing him to Forman’s self-pay facility.
On the morning of July 3, 2025, Forman reportedly contacted the family, stating Burton had been seen asleep around 5 a.m. but was missing by breakfast time (around 7 a.m.). He was last seen at the facility around 10:30 p.m. on July 2.
The facility was later shut down following the DHHS investigation. Reports indicated that even after being ordered to close, some residents were still found there in the following weeks.
Family Continues Search for Answers
As of the latest, Larry Darnell Burton remains missing. He is described as a Black male, approximately 5’9″, 160 lbs, with gray hair and hazel eyes.
