Unlicensed Facility Owner Could Be Charged After 74-Year-Old Man Goes Missing

A North Carolina family is still demanding justice and accountability after 74-year-old Larry Burton vanished from a Greensboro care home that was later confirmed to be operating illegally. Now, state investigators have asked Guilford County DA to bring charges against the owner of the unlicensed facility.

The North Carolina Beat spoke exclusively with Latonya Swift, Burton’s daughter, who says her father had been hospitalized in late June 2025 at Moses Cone. Swift says hospital staff refused to discharge her father home due to his medical condition, labeling him “combative” and saying he needed rehab — but insurance denied it because he could “walk over 450 ft.”

“They said we either pay $11,000 out of pocket or they’d transfer him to a facility that worked with self-pay clients,” Swift told The North Carolina Beat.

That facility is L&L Family Care Home on Moody Street in Greensboro.

Hospital Hand-Off to an Illegal Facility

Swift says her father was picked up and transported to L&L Family Care Home on June 24, 2025. On July 3 around 7:18 a.m., Swift said she got a call from Hazel Forman, who identified herself as the owner of the assisted living home.

Hazel Forman, the alleged owner of L&L Family Care Home in Greensboro, is under state investigation after a 74-year-old man went missing from her unlicensed facility. She may face criminal charges for operating without a license and possible elder neglect.

Swift said Forman claimed Burton had last been seen sleeping around 5 a.m., but by breakfast time at 7 a.m.— he was gone.

“They didn’t want us picking up my daddy from the hospital. They said it would set him back,” Swift said.
“But now, he’s missing — and that would’ve never happened if they just let him come home.”

Swift lives in Florida and rushed back to North Carolina after hearing the news. She said two of her children were with her father in the state, and the family had been taking care of him before the hospital refused to discharge him back to them.

“My daddy would’ve never been f**king missing if they had discharged him home,” she said.
“We’ve been taking care of him this whole time, but they wouldn’t do that sh*t.”

According to the North Carolina Department of Health and Human Services (NCDHHS), L&L Family Care Home was unlicensed. That alone is a serious violation. But now, with Larry Burton still missing, criminal charges may be filed against Forman.
